The PanCare Bus will be at Tolar next Tuesday, May 19th giving sports physicals to students who have completed the required documents. 

If you want your child to have a physical done at Tolar next Tuesday through Pan Care, please have them bring home a packet , fill it out and return it by Friday, May 15th. Once they bring back the packet 100% COMPLETE, they will be signed up for an appointment time slot. 

All forms must be fully completed, and every required parent/guardian signature spot must be signed. The EL2 form has two sections requiring both the parent and student signatures (Pages 2 & 4).

Please remember to keep track of the EL1, EL1/2S, and EL2 forms once the provider has signed them. If copies are needed, the parent/guardian must complete the Authorization to Release Medical Information form (attached in the packet) and email it to MedicalRecords@PanCareFL.org.

Once the completed authorization form is received, please allow up to 30 days for processing. Copies of requested forms will be sent via certified mail.

You may use your own doctor for a sports physical if you choose. Forms for that can be found in the front office.

No student will be allowed to participate in sports without an up to date physical.
